Health Passport


A Health passport is for children and young people in care and care leavers. It is important as it gives you details about your personal health history. It holds personal information about your health which can be accessed and updated at any time when you visit your GP or other medical services. The Health Passport is confidential as it is all about you. It has been completed by professionals about your health, any diagnoses, and medications that you may have had through your childhood.

The Health Passport can also contains useful information and provides advice and support on health issues e.g. Health and Well-being, Mental Health, Drugs and Alcohol and Sexual Health.

Some people feel the health passport needs to be updated so we’re asking for your views!
